Manyvids Co-Model Agreement help

So I have a manyvids which i've posted a few videos on, but my boyfriend and I have a few videos we made that we want to post as well. So I'm trying to fill out the co-model agreement form, but it wouldn't let me really fill it out? When I clicked on it, it brought me to another tab on my browser but I can't do anything to it. Help?

It looks like it needs to printed out and filled out the old-fashioned way, which is super annoying.
