Chaturbate performer agreement

Does anyone know how to get a copy of the agreement with my name on it.
I can't find it on the site, and they have not replied to my message.
I need it to set up an epayment account to get my money.

Having the agreement with our names on it, and possibly our address too, would also be nice so that it could be used as proof to show we are independant contractors (or whatever the proper term is) to Chaturbate.
